Founded by a healthcare professional with decades of experience in geriatric care and rehabilitation, our company provides personal consulting services, educational outreach, and technology solutions to help families and professionals support older adults across all care settings.

Our work is grounded in the 4Ms Framework of Age-Friendly Health Systems. The 4Ms is an evidence-based framework developed by the Institute for Healthcare Improvement (IHI). We encourage you to learn more about the 4Ms on their website www.ihi.org.

The 4Ms refer to:

  • What Matters – We take time to understand what matters most to each person we serve, ensuring that medical care plans, housing choices, and daily routines reflect their individual goals and preferences.

  • Medication – We support safe medication practices by helping families prepare for medical visits and work with care providers to understand prescriptions and avoid medication-related risks common among older adults.

  • Mentation (Mood and Cognitive Function) – Through early identification and supportive strategies for memory loss, depression, and confusion, we promote cognitive health and family awareness.

  • Mobility – Whether at home, at senior living facilities, or in the community, we promote safe movement and physical activity that fosters independence and reduces fall risk.

We integrate the best practices of the 4Ms framework into everything we do. From personalized family consultations to the development of AI-powered virtual assistant tools, Aging Well Ally bridges the gap between traditional care and modern solutions.
